HOW TO OBTAIN NY STATE LMSW GRANDPARENTING FORMS

  • Call NYSED at 518.474.3817 x-250 and request the “LMSW Licensing Application Package”
  • Submit an application (Form LMSW-1) and fee for initial licensure ($270) prior to September 1, 2005;
  • Have graduate school verify completion of the M.S.W. degree (Form LMSW-2);
  • Be at least 21 years of age and be of good moral character;
  • Have your employer(s) or licensed professional colleague(s) verify the completion of at least five years of full-time employment in social work prior to September 1, 2005 (Forms LMSW-4 and LMSW-4B).
  • Complete Form 5, LMSW Application for Limited Permit if planning to obtain LCSW.

Experience will be reviewed by the Department to determine if it is acceptable

Note: After September 1, 2004, a license or a limited permit to practice will be needed to practice as an .M.S.W. and plan to obtain the L.C.S.W. If all requirements for licensure are met under this provision and experience is being reviewed, it is possible to apply for a limited permit to practice with supervision.

Form 1CE Certificate of Exemption – Identification and Reporting Child Abuse and Maltreatment Training http://www.op.nysed.gov/form1ce.pdf - This form is not for all applicants. Use this form only if applying for an exemption to the requirement to complete training or coursework in the identification of child abuse and maltreatment because the nature of one’s practice excludes contact with children.
