WEAVING NETS UNDER THE CRACKS
INNOVATIVE SOCIAL WORK ROLES IN HEALTH/MENTAL HEALTH

  • Sandra Talavera, Coordinator, Senior Caring Program, Mount Sinai Hospital
  • Miguel Ortiz, Intensive Case Manager, Intensive Case Management Program, Federation Employment and Guidance Services

"Falling between the cracks" is an often repeated phrase as persons with health and mental health needs leave hospitals, prisons and other agencies or are trapped in their own homes without continuing care. Increasingly the needs of these people can be met by services that provide outreach and link them to continuing care. The three presentations at this meeting will illustrate three social work programs that "weave the net" that prevents very vulnerable people from falling between the cracks.

The presentations will be followed by discussion of:

  • job opportunities
  • educational resources
  • program availability
  • funding issues

We are looking for other models of social work practice for future forums.
