The Aging Services program at The Bridge (presenter’s organization) addresses critical gaps in the mental health and housing system by offering individualized care to seniors with behavioral and physical health conditions living in supportive housing. A mobile treatment team, the Aging Services program is comprised of a multidisciplinary staff that provides direct care to link older adults living in supportive housing with appropriate community-based services to manage age-related health challenges. The mission of the program is to identify and address barriers to aging in place for older adults living in supportive housing, with the goal of increasing community tenure and quality of life.

 

New York’s supportive housing system was not designed to support and advocate for older adults to continue to live in the community as their needs change. In this webinar, the presenter will explore how to support clients to stay in the community for as long as it is safe, while providing information to understand the current context of geriatric mental health.

 

 

With this workshop, participants will be able to:

  • Understand the landscape of aging in place with serious mental illness, using national and NY-specific statistics
  • Analyze the symptoms associated with aging with serious mental illness to better combat stereotypes and misinformation
  • Explore the shortage of geriatric mental health care providers
  • Understand the history and role of supportive housing and the increasing number of older adults living in supportive housing
  • Learn an overview of The Bridge's Aging Services program, which enables older adults living in supportive housing to age in place using a person-centered approach

Rebecca Heller, LCSW is passionate about the intersection of aging, mental illness, and housing. Currently, Rebecca is the Director of Aging Services at The Bridge, leading an innovative mobile treatment team that enables older adults living in supportive housing to age in place with an improved quality of life.

 

Previously, Rebecca worked at the West Side Federation for Senior and Supportive Housing in their assisted living residence.

NASW-NYC Certificates Policy

 

As per New York State Continuing Education guidelines, attendees must arrive on time and attend the ENTIRE workshop to receive their Continuing Education Contact Hours. Any attendees who arrive late or leaves early to a workshop will not receive their certificate, is not allowed admittance into the workshop and is also not eligible for a refund in any way. 

 

Please ensure that your registration information is accurate as this may impact your continuing education certificates.  NASW-NYC is not responsible for the accuracy of your registration information. To receive your certificate of completion, you must be present for the entire webinar and complete an evaluation.

 

 

NASW-NYC Refund Policy

 

All refund requests must be submitted by e-mail to accounting.naswnyc@socialworkers.org. Refund e-mails must include the subject line "Refund Request" as well as the attendee's name, workshop date and title in the body of the e-mail. Valid requests must be received no later than three business days prior to course date. NASW-NYC reserves the right to refuse any refunds requested later than three days prior to the course date.

  • All refunds are subject to a 30% administrative processing fee.
  • Refund requests typically require 1-2 weeks processing time.
  • Refunds ONLY; credit cannot be issued toward a future program date or substituted for another workshop.

 

If the workshop is cancelled, you will be informed ahead of time and fully refunded.
