As health reform incentives and penalties add pressure on hospitals to reduce length of stay, end of life decisions need to be made more quickly, yet the choices and options available to patients and families are getting more difficult to understand. Today 85% of people want to die at home yet only 45% do, pointing to an opportunity for social workers to advocate for the their needs and preferences by providing non-directive counseling about their care continuum options and implications.
This workshop will outline an innovative approach to advance care planning that involves identifying, communication, and incorporating personal preferences and priorities into current and future decisions about care. Three case studies will be shared that demonstrate how social workers effectively educated difficult families about disease trajectory, treatment options and resources to improve quality of life.
The presentation aims to spark dynamic dialogue with participants that touch on advocacy, ethics, community resources, legal and regulatory implications, and more.
Eileen Zenker, LCSW, CCM is a NYU School of Medicine instructor for bioethics. Eileen has experience teaching at the National conference speakerships including the Society of Social Work Leadership. Additionally, Ms. Zenker has served as a webinar speaker for a national elder care seminar series.
